• We practice routine gynecologic care in women who have just begun cycles throughout their reproductive years and into menopause.
• We offer annual well-woman exams, routine health screenings, and specialty care in pelvic pain, infertility, and other women’s health concerns. We also offer hormone replacement therapy.

• We do not require that you chart your cycles prior to your first appointment. However, charting helps us to see more clearly the signs that your body is revealing. Documentation of symptoms, cervical mucus, and bleeding is very important for your gynecologic health.
• For women with regular cycles, we do need two cycles of charting before ordering targeted hormone labs.

• The Creighton Model Fertility Care System is the cornerstone of NaProTechnology.  We encourage you to learn this charting system.

NaProTechnology is a women’s health science that searches for and treats root causes. We do a comprehensive workup to determine all of the diseases that are affecting you. Together we create a treatment plan to restore your health- mind, body and soul. Our thorough approach and careful attention aims to optimize health and fertility. No one leaves Vivify with the diagnosis of “unexplained infertility.”

Although Dr. Pakiz has delivered over 1500 babies, she chose to set aside her obstetrical skills to focus on treating gynecologic health diseases (such as endometriosis) since the need is so GREAT. Vivify does provide care for women who are newly pregnant (up to 13 weeks gestation). This includes pregnancy hormone monitoring, progesterone monitoring, and early ultrasound to confirm the baby’s health.

• You do not need a referral letter to be seen at Vivify, but you are welcome to fax one. Our fax number is 866-282-8063.
• When you schedule your appointment, please inform our receptionist that there are records you would like to send prior to your appointment. A release of records form will be sent through the patient portal.

All three hospitals are part of the CHI Health system. We recommend calling your insurance company to ensure that Dr. Pakiz and the hospital are in-network with your plan.

We have cash pay rates for those who do not have health insurance or are out-of-network for their insurance plan. We require that such patients prepay for their appointments.
